Please complete all patient forms before your visit to ENT Carolina.
You will need to complete our pre-registration forms online in the Patient Portal. If you did not receive a user name and password, please contact us so we can assist you. Completing your patient questionnaire before your appointment will expedite the check in process.
If you are unable to access the Patient Portal, you may complete the questionnaire when you arrive at our office. We recommend that you arrive 30 minutes before your appointment time. You will not be able to see the provider until the patient questionnaire has been completed.
