Please allow at least one hour for your initial appointment.

If an emergency arises, we will do our best to notify you before your appointment; however, please be aware that is not always possible.

What to Bring

Pre-Appointment Checklist
  1. Completed pre-registration online
  2. Name, address, and phone number of your pharmacy
  3. Medication list
  4. Insurance card(s)
  5. Driver’s license
  6. Any previous medical records, especially test results and X-rays (if needed)

Arrive early to complete paperwork

Please plan to arrive 10 minutes before your appointment time. Please arrive 30 minutes before your appointment if you need to complete paperwork at our office. If you have custody of a minor child, you MUST bring legal documentation to the child's appointment.
